  • After the initial 6 months, memberships may be canceled with 30 days written notice

  • If you join on the same day as a procedure, your first payment will be banked and applied to a future visit

  • Membership discounts may be combined with Aspire, Alle, and Merz rewards programs. Our team is happy to help you enroll and manage these accounts.

  • Monthly membership balances cannot be supplemented with promotional or vendor credits. Rewards programs may still be applied at checkout.

Subscriptions are an initial 6-month commitment. We encourage a 12-month commitment to achieve a complete correction. No cancellations in the first 6 months. Your first month’s subscription contribution will be due prior to receiving membership benefits. Your initial contribution can be paid the same day as your procedure, however the membership dues will be banked for your next visit. Subscription payments are non-refundable. Subscription discounts may be combined with Aspire, Alle and Merz rewards programs. Our office will help you sign up for and manage rewards accounts. Monthly subscription balance cannot be supplemented by any other promotional vendor programs such as Alle (Brilliant Distinctions), Aspire Rewards, loyalty points or account credit, though these programs can be used at checkout. You may cancel your subscriptions at any time after 6 months with 30 days’ notice. No refunds will be given. You will be able to spend all of your funds in the office. Discounts are applied at time of service, not retroactively. The practice reserves the right to update the list of excluded services with notice, to protect pricing integrity and consumable costs.
